We Operate
The Domiciliary
Care Co-ordinator is contacted by Care Managers from NHS Trust,
Social Services Trust or by relatives for Private Individuals,
who provide information about potential clients. This includes
where the person lives, type of service required, special needs
which the individual may have, available support from family
members and other agencies. A starting date for receipt of the
service will also be arranged.
The client is then
included on a rota and care staff are informed and given brief
details about the needs of the person and a start date. The Co-ordinator
or Care Service Manager will visit the client and meet with
their family members to see if there are any issues that need to
be addressed at this stage.
If there are any
underlying issues, the Co-ordinator or Service Manager will
liase with any parties involve and can provide professional
guidance. The dedicated teams at Carefirst 24 work very close to
all members of multidisciplinary team. |
