Nurse Team Leader / Nurse Specialist (Mental Health - Community)Band 6
Job Summary
• Assesses patients, plans and implements individual care programmes and therapeutic interventions in a range of community mental health settings including the home, clinics and intermediate care.
• Provides specialised advice in the community and maintains associated records
• Liaises with other agencies in planning and assessing programmes of care
• Provides individual support for people with a mental health problem to enable them
to live in the community and make their own choices
• Works within an interdisciplinary team to deliver quality care with an emphasis on
promoting independence and rehabilitation
• Carries out nursing procedures
• Provides clinical supervision to other staff, students
Training / Qualifications / Knowledge
Registered on the appropriate part of the NMC register for the assignment RN (Mental Health ) RN3, RN4. Relevant diploma level specialist training or equivalent as required for the assignment.
Evidence of continuing professional education and also Evidence of professional updating
Desirable Criteria: Teaching & assessing qualification + Phlebotomy
Experience
Minimum of two years post registration experience. At least 1 year (within last 6 months) in the field of the assignment. Crisis management and intervention
Skills, Abilities, Knowledge and understanding about the above position will be asked at interviews
