Nurse Team Leader / Nurse Specialist (Community). Band 6
Service Users:
Members of the community– in the home, in clinics and in intermediate care settings.
Job Summary
• Assesses patients, plans and implements care in the community.
• Provides skilled, professional assessment of health needs, relevant nursing care and health information to members of the community
• Provides advice and maintains associated records
• Carries out nursing procedures
• May provide clinical supervision to other staff, students
• Provides clinical leadership to the community nursing team and co-ordinates nursing team work loads
• Organises community placements for students, provides induction training, clinical supervision
• Works in partnership with the wider primary care team and social cervices to deliver quality care with an emphasis on promoting independence and rehabilitation as well as curative and palliative care
• Works within codes of practice and professional guidelines.
Training / Qualifications / Knowledge
Registered on the appropriate part of the NMC register for the assignment RN (Adult) / RN1/ RN2 OR RN (Child)/RN8. Relevant diploma level specialist training, or equivalent as required for the assignment. Evidence of professional updating.
Desirable Criteria:
ENB 998 or equivalent Teaching & assessing qualification.
District Nursing Qualification.Qualified Nurse Prescriber .Health education qualification.
Experience
Minimum of two years post registration experience. At least 1 year (within last 6 months) evidence based practice caring for people in the community in a health care setting appropriate to the field of the assignment.
