Nurse Team Manager (Community). Band 7
Service Users:
Members of the community– in the home, in clinics and in intermediate care settings
Job Summary:
• Manages team of community nurse specialists and other staff covering a geographical area including recruitment and appraisal.
• Assesses patients, plans and implements care in the community.
• Provides skilled, professional assessment of health needs, relevant nursing care and health information to members of the community
• Provides advice and maintains associated records
• Carries out nursing procedures
• Provides clinical leadership to the community nursing team and co-ordinates nursing team work loads
• Organises community placements for students, provides induction training, clinical supervision, works in partnership with the wider primary care team and social cervices to deliver quality care with an emphasis on promoting independence and rehabilitation as well as curative and palliative care.
Training / Qualifications / Knowledge:
Registered on the appropriate part of the NMC register for the assignment RN (Adult) / RN1/ RN2 OR RN (Child)/RN8. Professional knowledge acquired through degree supplemented by diploma levels specialist training , experience, short courses., as appropriate for the assignment. Evidence of professional updating ENB 998 or equivalent Teaching & assessing qualification District Nursing Qualification.
Experience:
Minimum of four years post registration experience .At least 2 years experience within the field of assignment. Current (within last 6 months) evidence based practice Staff management and supervision.
